Amber Kapinos - District Nurse
Professional experience:
I have been a Registered Nurse for over 15 years within a variety of settings; including general medicine, orthopedics, urology, oncology, pre-surgical and post surgical. I also have professional experience as a health educator as well as a nurse educator.
I am married with two children. We love to be active within the Denmark community whether that be attending sporting events, concerts, or theatrical performances--Denmark is a GREAT place to raise a family.

This form needs to be filled out for all medications that may be given at school. If your child has a prescription medication (i.e inhaler, EPI pen, ADHD medications) this form needs to be filled out and signed by a physician. Your physician’s office can fax the form back to the appropriate school or you can drop it off at the school office.

If your child will have a medication such as Tylenol, Advil or Benadryl at school, please complete this form (no need for a physician signature) and send completed form back to your child’s school along with the medication that you are requesting to have on hand.
*REMINDER - a new form needs to be completed at the beginning of every school year
